Liability & Media Release
I acknowledge that participation in tennis, pickleball and related fitness, conditioning, and Pilates activities organized by 3015 Tennis Academy LLC involves inherent risks, including injury, illness, accidents, falls, collisions, equipment failure, and other hazards. I voluntarily assume all risks. I release and hold harmless 3015 Tennis Academy LLC, its staff, coaches, personal and fitness trainers, conditioning coaches, Pilates instructors, affiliates, Hoover High School, Glendale Unified School District, and the City of Glendale, including their officers, employees, and agents, from any and all liability for claims arising from participation, including those resulting from negligence. I agree to indemnify and defend the parties listed above against any claims brought by third parties related to my (or my child's) participation. In the event of a medical emergency, I authorize 3015 Tennis Academy LLC to seek treatment on my behalf (or my child's). I understand this agreement is governed by California law. Media: photos and videos taken during activities may be used for promotional purposes (website, social media, print). If you prefer to opt out of media use, please notify 3015 Tennis Academy LLC in writing by email prior to participation (include participant name). Your media opt-out will remain in effect until revoked in writing.
Parent Court Policy
Parents are not permitted on the court while children are practicing. We have a designated viewing area for parents to sit and watch, or parents may choose to drop off their child and return for pickup at the end of practice.
Cancellation Policy
24HR Cancellation Policy
- You must cancel 24hr ahead of time or you are subject to a late cancellation and will lose that class or private lesson.
Monthly Membership
- Classes do not roll over and expire 28 days from purchase.
- Your purchase date is your start date for each month thereafter.
- You must email a 28 day cancellation request 28 days prior to your next month's billing date to cancel membership.
Drop-In's
- Expires 28 days from purchase date.
Returns and Refunds
No refunds or exchanges on any purchases. No sharing or splitting of any classes, privates, drop-in's, memberships or packages.
Membership Pause Policy
Members may request a temporary membership pause due to travel, medical reasons, personal scheduling conflicts, or other approved circumstances.
- All pause requests must be submitted in writing by email to info@3015academy.com and must include the reason for the requested pause.
- Pause requests should be submitted at least seven (7) days prior to the requested pause start date whenever possible.
- Membership pauses must be a minimum of fourteen (14) consecutive days.
- Memberships may be paused for a maximum total of two (2) months per calendar year.
- Medical-related pauses require a valid doctor's note or medical documentation stating the member is temporarily unable to participate in tennis activities.
- Memberships may only be paused a maximum of two (2) times per calendar year.
- Membership pauses are not retroactive and cannot be applied to past billing periods.
- Membership accounts must be current and in good standing in order for a pause request to be approved.
- Members approved for a pause will retain their current membership rate and academy placement during the approved pause period.
- Pause requests extending beyond the maximum allotted pause period may be subject to a membership hold fee, determined at the sole discretion of 3015 Tennis Academy.
- Membership billing will automatically resume at the end of the approved pause period unless otherwise communicated in writing by 3015 Tennis Academy.
- Unused pause time does not roll over into future calendar years.
- Missed classes, lessons, or training sessions outside of an approved pause period are not eligible for refunds, credits, or extensions.
- 3015 Tennis Academy reserves the right to approve, deny, modify, or discontinue pause privileges at any time and may request additional documentation when necessary.
Our goal is to provide reasonable flexibility to members while maintaining fairness, scheduling consistency, and program availability for all academy participants.
